The slightly confusing naming convention is due to the fact that Shin Megami Tensei IV: Apocalypse is set in the same post-apocalyptic world as the prior game, but in an alternate timeline (and featuring a new main character). With the previous entry being such a huge hit both critically and commercially, Shin Megami Tensei publisher Atlus unsurprisingly released a sequel three years later, again on the 3DS.
